Please provide us with 48-hour
notice of cancellation if you cannot make
your scheduled appointment.
If an appointment is broken,
or if there is a late cancellation,
a fee may be charged.
PAYMENTS AND INSURANCE
INFORMATION
In addition to providing excellent dental health care,
we are also concerned with making it cost effective for
you. We accomplish this is by eliminating expensive billing
services and ask that you provide payment at the time
of your visit. For your convenience, we accept, Master
Card, Visa, checks as well as cash.
All financial arrangements
will be made at the time of service.
We offer 3 methods of financing:

DENTAL
INSURANCE
For
our patients with dental benefit plans, we will submit
necessary pre-determination and claim forms, and other
information to your insurance company. We are happy to
cooperate with you and your insurance carrier to insure
that you receive the maximum reimbursement for your procedures.
Any balance that remains
outstanding 30 days after completion of treatment,
for which financial arrangements have NOT been made, will be charged an
interest rate of 1.5% per month.
